  1. Full Name
  2. Name of War, Tour and/or Area of Service
  3. Dates/years of Service
  4. State the assigned duties
  5. State the branch of service and any Awards received.
  6. Send a short biography about the veteran’s place of birth, education, and the time lived in Concordia Parish.
  7. Include a picture of the veteran in uniform.
